Healthy Hearts, a collaboration with Providence Seaside Hospital, Sunset Empire Park and Recreation and the Clatsop County Community Advisory Council for Columbia Pacific CCO, presents a 12-week program beginning in January.

The program coordinator works with each individual on a one-on-one basis. The coordinator and participant decide on weekly goals and talk about how they can reach them. The coordinator has a wide range of free resources available including classes and activities at Sunset Empire Park and Recreation.

Healthy Hearts is open to all Oregon Health Plan members with a Providence primary care provider. To sign up for the program, email Sarah Lacoste-Brown at lacostebrowns@careoregon.org.
